Important Information
This is a reward application request and is not valid for transportation. Rewards tickets must be requested via the 7th Heaven Department and require ticketing prior to travel. Airport ticketing is not permitted. Reward tickets for travel issued less than 7 days prior to departure date will be subject to a rush processing fee. Chairman's club members may access rush service at no extra cost. Third-party reward requests must be accompanied by a signed letter from the frequent flyer and faxed to (305) 669 6631 USA or (876) 929 5658 JCA along with a copy of the 7th Heaven member's valid ID. Applicable government taxes /fees may apply. Please contact the 7th Heaven Department or Reservations for applicable charges.
